Listed below are some of the forms and applications used by The University of Iowa Office of Student Financial Aid. Please check that you are selecting forms for the appropriate summer or academic year period.

Free Application for Federal Student Aid (FAFSA)--This form must be completed by U.S. students who are applying for financial aid awarded by the Office of Student Financial Aid.
Federal Verification Form--If your FAFSA was selected for federal verification, this form is available in the Application/Loan Document Status section in MyUI.
Summer Financial Aid Application--The summer application is available in MyUI and must be completed by students applying for financial aid for the summer session. Students must be registered for their summer classes before completing this form.
Winter Financial Aid Application--This form must be completed by students applying for financial aid for the winter session. Students must be registered for their winter class before completing this form. The winter application will be available when winter registration begins in November.
Aid Adjustment Form--Decline or reduce your loan(s) on the Student Loan Confirmation page in MyUI. Complete the form below if you are requesting that your other financial aid be reduced or cancelled.
Changes in Enrollment Hours and/or Residency Status--These changes can be made in MyUI. On the Award Notification screen, click on Send Corrections to OSFA.

Master Promissory Note (MPN) for Federal Direct Loans--Complete this promissory note after you have been awarded a Federal Direct Loan. This is not an application for a loan.
Master Promissory Note (MPN) for Federal Perkins, Nursing, and Health Professions Loans--Complete this promissory note after you have been awarded a Federal Perkins, Nursing, or Health Professions Loan. This is not an application for a loan.
Request/Application for PLUS (Parent) Loan--Parents of dependent students must electronically complete this request/application, on the studentloans.gov website, each semester/session (fall, winter, spring, and/or summer) they want to accept the PLUS loan that was offered on their student's award notification letter.
- The request/application for the Fall 2017/Spring 2018 semesters will not be available to complete until mid-April 2017.
- When on the studentloans.gov website, click on "Parent Borrowers" then "Apply for a PLUS Loan." Separate requests must be submitted for fall and spring semesters.
- Until this request/application form is processed, the Office of Student Financial Aid will not proceed with the parent loan even if there is a valid MPN on file.
- If a parent completes the PLUS request on the studentloans.gov website, they should not complete a paper form (below).
- If a parent is not able to complete the electronic request, they may complete a paper form (below). Be sure to complete the form for the correct loan period.
Consortium Agreement Form--This form is for students who will be taking courses simultaneously at the UI and another domestic institution and want to combine those hours to fulfill the registration hour requirement for financial aid disbursement.
Non-Degree Student Status Form--This form is for UI non-degree students who are required to take pre-requisite courses to be considered for admission into a degree program.
Release of Information--This form is for students to provide authorization for the UI Office of Student Financial Aid to speak or correspond with another person/agency about their financial aid.
Teach Grant Application--This form is for College of Education students to apply for TEACH Grant Funds.
Submitting Scholarship Checks--Information is available on the University Billing Office website.
